Eszopiclone

Insomnia (sleeping problems) is a big problem especially in western countries, as it affects 6% to 10% of the population over there.

Lunesta is one of the new drugs that deal with insomnia effectively and with a lower chance of dependence.

Compared to Benzodiazepines, it has the same efficacy and lower chance of abuse and dependence.

Eszopiclone belongs to a drug group called the Z-drugs; it contains similar hypnotic drugs like zaleplon and zolpidem, but it has superiority to them that it can be used for a relatively long time than the other group members.

Eszopiclone is rapidly absorbed after its administration, and it reaches the peak after about 1 hour.

Indications:

Eszopiclone is used in treating transient and chronic insomnia.

lunesta is considered to treat them by decreasing the time for sleep induction, giving better sleep maintenance, giving longer sleep duration, and better quality.

It is also superior to benzodiazepines in next-day functioning.

It helps in calming the brain which helps to improve sleeping problems.

Dose and administration:

lunesta is found in 1, 2, and 3 mg tablets.

Pediatrics: 

There are no established data on eszopiclone in pediatrics, that’s why eszopiclone is not used in pediatrics.

Adults:

Patients are starting with 1 mg daily before bedtime.

This dose can be increased to 2-3 mg daily if the medical condition requires so.

Higher doses are more likely to increase the post-morning drowsiness.

It has to be taken immediately before going to bed.

Eszopiclone should not be taken with or after a fatty meal as it may interfere with the absorption of the drug.

You must have 7-8 hours to sleep after taking the drug, if you had to wake up before this period, you may experience sleepwalking which can be extremely dangerous.

Post-morning driving or operating heavy machines shouldn’t be done before knowing the response to the drug.

Problems with driving, memory, or coordination last up to 11 hours after taking the medication.

Dose adjustments can be made with other medications like CNS depressants.

No dose adjustments are needed for mild to moderate Hepatic and renal impairments.

Geriatrics:

The starting dose is 1 mg daily taken before bedtime.

It can be increased to 2 mg maximum.

Higher doses are more likely to cause post-morning drowsiness.

Adverse effects:

Common (more than 10% of drug users):

• Stuttered speech
• Nervousness
• Nerve pain
• Itching
• Rash
• Drowsiness

 Rare (less than 1%):

• Agitation
• Losing hair
• Swelling of the skin
• Asthma
• Eating disorder
• Bladder inflammation
• Swallowing difficulties
• Fever
• Nose bleeding
• Hypertension
• Hostile behavior
• Increase in cholesterol levels
• Decrease in potassium levels.

• diazepam intranasal
• dichlorphenamide

• lopinavir

Lunesta is very similar to zopiclone, be careful about the concomitant use of both of them.

Drug-alcohol interactions:

Consuming alcoholic beverages with eszopiclone increases the risk of side effects especially those related to drowsiness, sleepiness, and others.

Drug-food interactions:

Eszopiclone should not be taken with or after a fatty meal as it may interfere with the absorption of it.

Precautions:

       

   Black box warning:

 Eszopiclone may be associated with sleepwalking, sleep   

 driving, and performing several activities while not fully awake.

 These effects may be a reason for a really dangerous  

 consequences including serious injuries and may even lead to 

 death.

• Don’t take a higher dose of eszopiclone without the doctor asking you to, because higher doses increase the risk of serious side effects.
• If you have a severe hepatic impairment, the dose should not exceed 2 mg daily.
• Eszopiclone is usually used from 1-2 weeks, if you need the drug for more; consult with your doctor for more investigation as insomnia may be an underlying reason for a more serious medical condition.
• If you miss a dose, don’t take it and take the next one, don’t double the doses to not suffer from overdose side effects that may lead to coma.
• Eszopiclone may cause suicidal thoughts and mood changes, be aware of any change on your thoughts and contact your health care provider at once.
• Eszopiclone can cause serious allergic reactions, notice any symptoms of allergy like swelling, trouble breathing, nausea, and vomiting.

Eszopiclone is effective with fewer side effects than benzodiazepines.
